Who we are
The NeuroHome Shop is operated by NSH Operations, LLC, doing business as NeuroHome, 780 Bayou Pines East Dr, Lake Charles, LA 70601. In this policy, "we", "us", and "our" mean NSH Operations, LLC.

What this policy covers
This policy describes how personal information is collected, used, disclosed, and retained when you visit or buy from the NeuroHome Shop. It applies to the Shopify storefront and commerce activity connected to it.
Intake, assessments, session records, laboratory results, care records, and other information collected in connection with program delivery are handled outside the Shopify storefront under the privacy notices, consents, and terms that apply to those systems and services.
If a Notice of Privacy Practices applies to care you receive, that notice is separate from this storefront privacy policy.
Information we collect
Depending on how you use the store, we may collect:
• Contact information, such as your name, email address, telephone number, billing address, and shipping address.
• Order and transaction information, including the products or programs selected, amounts paid, discounts, refunds, and order status.
• Payment-related information. Payment card details are processed by the applicable payment provider; we do not intentionally store your full payment card number in our own systems.
• Customer-service communications and information you choose to provide when contacting us about an order.
• Technical and usage information collected by Shopify and enabled services, such as IP address, browser and device information, cookie identifiers, pages viewed, referral information, and security or fraud signals.
Health information and program enrollment
The Shopify storefront is not intended to collect diagnoses, medical history, assessment responses, laboratory results, care notes, or other intake information. Please do not place that information in order notes, product reviews, storefront messages, or other Shopify fields.
If you purchase or request information about a NeuroHome program, authorized staff may use your order and contact information to begin enrollment. Commerce or enrollment information from the order may later be associated with an enrollment or care record maintained outside Shopify. Intake and care documentation occur in the systems designated for those activities, not in the Shopify storefront.
How we use personal information
• To process and fulfill orders, provide order updates, and respond to customer-service requests.
• To initiate program enrollment when a program is purchased or requested.
• To operate, secure, troubleshoot, and improve the storefront.
• To detect or prevent fraud, misuse, or security incidents.
• To comply with legal, tax, accounting, regulatory, and recordkeeping obligations.
• For other purposes disclosed at the time information is collected, or with your consent where required.
How we disclose personal information
We disclose personal information as reasonably necessary to operate the store and fulfill transactions. Recipients may include:
• Shopify, which hosts and operates the commerce platform and processes customer data under its applicable terms and data-processing arrangements.
• Payment processors and financial-service providers that process transactions.
• Shipping and fulfillment providers that deliver physical orders.
• Accounting, tax, legal, insurance, security, information-technology, and other professional or operational service providers.
• Government authorities, regulators, courts, or other parties when disclosure is required by law or reasonably necessary to protect legal rights, safety, or the integrity of the store.
• A buyer, successor, or adviser in connection with a merger, financing, reorganization, sale, or transfer of some or all of the business, subject to applicable law.
We do not sell personal information for money. Whether a particular disclosure is treated as a "sale" or "sharing" under a state privacy law can depend on that law and the technologies enabled on the store. Where applicable law provides an opt-out or other privacy right, we will honor it.
Cookies and similar technologies
Shopify and enabled store services may use cookies and similar technologies for functions such as cart and checkout operation, security, fraud prevention, preferences, analytics, and, if enabled, marketing. Where applicable law requires consent or an opt-out mechanism, the store will provide the required choice.
How long we retain information
We retain personal information for as long as reasonably necessary for the purposes described in this policy, including order support, returns, accounting, tax, fraud prevention, dispute resolution, and legal obligations. Retention periods may vary by category of information and applicable law.
Your privacy choices and rights
Depending on where you live, you may have legal rights to request access to, correction of, deletion of, portability of, or restrictions on certain uses of personal information, and in some jurisdictions to opt out of certain sales, sharing, or targeted advertising.
You may submit a privacy request to shop@neurohome.app. We may need to verify your identity before completing a request, and some information may be retained where permitted or required by law.
Children
The storefront is intended for adult purchasers. We do not knowingly invite children under 13 to create accounts or submit personal information through the store. NeuroHome programs may serve children, but program enrollment and purchases are completed by a parent, guardian, or other authorized adult.
Changes to this policy
We may update this policy as our practices, technology, or legal obligations change. The updated version will be posted with a revised "Last updated" date. Material changes will be communicated as required by applicable law.
